FTA has several forms that must be completed by new employees, ranging from tax forms to forms necessary for badges. Please fill out these forms prior to arriving at the work site; this will save time in bringing new employees aboard; refer to the Required Forms section of this page for details. In addition, several documents detailing various Federal programs and benefits are listed in the New Employee Orientation navigation menu on the right-hand side of the page.
The New Employee Orientation Form linked here are required for employment with FTA and are in PDF format. These forms are available for you to fill out at your local computer, print them, sign them, and turn them in upon physically reporting to FTA. The forms will auto-fill fields that repeat (such as name), once they are initially filled out.
Please note that the PDF forms will not perform validation on the data you enter; you are required to verify that the information is accurate prior to signing the printed document. Access to these forms is limited to filling out and printing only; users will not be able to save the completed forms unless they have the full version of Adobe Acrobat. The data entered in these forms will not be saved or transmitted in any way.
